There is also a location for who is being billed for the work and a detailed line item description is included with an indication if it is taxable or not along with a dollar line amount. Subtotal, taxable amount, tax rate, total tax due, shipping fee, and the total amount due are all tallied at the bottom of the form.

Specifications you may want to include on IT forms include: Hourly rate; Total number of hours worked; Hardware costs; Consultant costs; Date payment is due.
Make sure to state in the comments any specific hardware issues that were found during inspection. It is typically beneficial to include any quotes of other issues found.
