Weekly Time Card

This form provides a standard time card layout to document the hours worked
by an employee. Fields are provided for payroll information such as the
employee's name and the start and end of the pay period. Each day of the
week includes fields for time in and time out along with the number of hours worked for the day.
Fields to total the week's hours, including regular and overtime are provided
along with approval signatures. See the monthly time card for a similar
version with four weekly periods.
